Case Manager – Our Journey Forward

Position Summary:

The Case Manager will work alongside maternity home clients as they establish their goals and will provide personalized, compassionate support to residents of our maternity home. This position will involve assessing client needs, developing care plans, connecting clients with appropriate resources, and offering ongoing guidance. The ideal candidate will demonstrate strong organizational, listening and case management skills, as well as a commitment to supporting the emotional, physical, and spiritual well-being of our residents. This role requires a passionate individual who desires to serve others through Christ’s love and empowering women facing life’s challenges.

Although this position reports to the Executive Director, the Case Manager will have a key working relationship with the House Mom and be a part of the Program Team.  

    • Client Assessments & Care Planning: Conduct comprehensive assessments of residents’ physical, emotional, and spiritual needs. Develop individualized care plans in collaboration with residents, addressing goals related to parenting, health, education, employment, and spiritual growth.

    • Resource Coordination: Connect residents with vital resources, including healthcare services, parenting classes, financial assistance, housing, and community programs. Maintain a network of community partners to facilitate residents' needs.

    • Mentorship & Counseling: Provide one-on-one emotional and spiritual support to clients, offering mentoring, guidance, and encouragement. Foster a nurturing and empowering relationship that supports personal growth, emotional healing, and faith development.

    • Documentation & Reporting: Maintain accurate and timely case files, including notes on client progress, goals, and outcomes. Maintain client stats and prepare reports for internal use and funding purposes. Utilize eKyros: Centerpiece (client management software) to record case activity on each client.

    • Collaboration: Work closely with other staff members to ensure a cohesive and collaborative approach to client care. Participate in regular team meetings to review cases and discuss best practices.

    • Crisis Intervention: Respond to crises as they arise, offering immediate support and coordinating appropriate interventions, including referrals to mental health professionals or emergency services.

    • Program Evaluation & Improvement: Participate in evaluating program outcomes and suggest improvements to better meet the needs of the clients and the community.

    • When needed, interview potential clients. Conduct assessment and intake of new clients.

    • In partnership with the House Mom, orient new clients to the program.

    • Support the House Mom to coordinate and oversee client attendance at work, school, or programs.

    • In partnership with House Mom, assess and monitor parenting skills with clients with appropriate intervention, counseling, education, and support. Assess children’s needs and establish plan with client to ensure physical, emotional, behavioral, and social needs of children are being achieved.

    • Train and supervise volunteers and interns as needed.

    • Attend weekly staff meetings and house meetings (as needed or required).

    • Represent R2H professionally at all times. Attend community networking meetings as a R2H representative to develop relationships and resources within the community.

    • Provide leadership in the house when you are the person overseeing the program component. Communicate any problems or concerns to the Executive Directors or House Mom as appropriate.

    • In the absence of the House Mom, lead the weekly house meeting.

    • Transport clients as needed in provided program vehicle.

    • Education and Experience:

    A bachelor’s degree and strong knowledge of practically applying biblical truth to problems in life, as well as experience with case plan management or client advocacy is preferred. Experience working with at-risk populations, and knowledge of pregnancy, child development, and community resources is a plus.

    • Skills & Competencies:

      • Strong problem-solving skills and ability to work collaboratively.
